2021: the year in review

When organizations came together to form ARCHI in 2012, they identified 7 critical issues that, if tackled collaboratively, could significantly improve health outcomes, reduce healthcare costs, and grow the metro Atlanta economy. While all these issues remain essential to health transformation, person-centered care coordination that addresses the socioeconomic needs that are essential to well-being has risen as a central strategy in ARCHI’s work.

ARCHI and its partners seek to Invert the Burden of navigating complex and siloed services in order to build a system of care that meets people where they are, centers individuals and families, and addresses the full range of issues and concerns to not just deliver services, but to support and empower Atlantans to stabilize and ultimately thrive.

Throughout 2021, ARCHI was a thought leader, driver of innovation, and facilitator of systems change to Invert the Burden in Atlanta.

Community Resource Hubs

ARCHI partners work collaboratively to address socio-economic needs of patients with chronic conditions. The majority of patients in the Community Resource Hub lowered or stabilized their blood pressure and A1c scores. Patients also achieved success in meeting housing, food, transportation and employment needs.

Re-designing for Person-Centered Care

ARCHI partners are working with resident advisors and technical experts to re-imagine how metro Atlantans access rental and financial assistance and how health systems connect patients to community care.

Housing continues to be the most significant unmet social determinant of health and the resulting housing instability the most significant driver of poor health outcomes.

COVID-19 Vaccine Support

ARCHI partners came together across a number of opportunities to provide targeted support to accelerate access to vaccines among people of color. This included direct vaccine support, education and outreach and shared learning among partners to rapidly apply innovative and community centered strategies.

Through our partners, ARCHI has accomplished hundreds of vaccinations and 1,000 vaccine registrations. As a collaborative we have reached over 26,000 people through face-to-face discussions, webinars, printed materials, and campaign initiatives.

Well-being in Atlanta

ARCHI and its partners explored how Atlantans define well-being this far into the pandemic and how it might shape policies and practices moving forward.

Atlantans define well-being as the ability to sustain oneself and affect change in their community. Economic opportunity and empowerment are essential to health for Atlantans.

Systems Assessment and Change

18 organizations worked with ARCHI to assess their own processes and found great opportunity for change because on average:
• They have to use 3-4 systems to collect and process client information
• They spend 60% of their time on administrative work
• Current processes were described as time consuming, confusing, inefficient and causing burnout

Real-Time COVID-19 Learning

ARCHI partners meet regularly to share successes and challenges as they work to reach out to populations particularly vulnerable during the pandemic. This real-time learning was and continues to be essential in the dynamic environment of the pandemic.

Partners maintained a consistent focus on populations with low vaccination rates including communities of color. Together they were able to explore together and share communication and outreach techniques, local and national resources to build community trust and increase vaccination rates.
